In Edit File Setting, check "Allow Multiple Upload Box in a product page" checkbox in the Advanced settings section. Make sure the product has more than one file setting targeting it.
Yes. When you enable Multiple Upload Box, you can set the order in "Upload Boxes Order" in the Edit File Setting page.
Yes, you can change the upload display name "Field Display Name" and the URL field name "Upload URL Field Name" in the Edit File Setting page.
Yes, you can change the file name in "Prefix/Suffix/Replace Text", the file name will always has a suffix of some numbers to prevent duplicate file names.

If you uninstall the app, you might still have a charge for the current bill period, but you will not be charged for the next bill period.
